ABOUT THE PROGRAM
The customer agrees to receive an A&D Medical Professional Office Blood Pressure Monitor from Medical Device Depot for a 30-day trial, pursuant to the following terms:

TRIAL PERIOD
The 30-day trial period beings on the date of delivery of the unit.

PURCHASING
At the end of the 30-day trial, the customer will be billed for the product.

RETURNING THE TRIAL PRODUCT
As an alternative to purchasing the trial unit, the unit may be returned to A&D Medical without cost or obligation. To return the trial unit, please contact A&D Medical at (888) 726-9965 to receive a Return Materials Authorization (RMA #) and return shipping information. The RMA number must be displayed on the outside of the return box. Please allow 1-4 business days for the return delivery of the trial unit to A&D Medical. Returns will not be accepted unless a Return Material Authorization (RMA) Number has been issued from A&D Customer Service Representative.

SHIPPING COSTS
A&D Medical agrees to pay all costs associated with shipping the trial unit to and from the customer site providing that all requirements of this agreement have been followed. If the requirements of this agreement have not been followed, A&D Medical relinquishes its responsibility for the payment of shipping costs, and shipping costs will become the customer’s responsibility.

FAILURE TO RETURN UNIT
If the trial unit is not returned to A&D Medical within two (2) weeks of the end of the trial period, Medical Device Depot reserves the right to convert this agreement into a binding purchase order automatically. Medical Device Depot will invoice the company named below for the full retail value of the evaluated product.

DAMAGE TO THE UNIT
The customer assumes responsibility for any damage caused to the trial unit when used in any capacity other than normal use. This may include, but not limited to, dropping the unit, water damage, operating the unit in a manner not specified by the operator’s manual, or attempted repair not authorized by A&D Medical.

LIMITATIONS OF LIABILITY
Medical Device Depot and A&D Medical disavow all liability for any incidental, direct, indirect, punitive, actual, consequential, special, exemplary, or other damages, including loss of revenue or income, pain, and suffering, emotional distress, or similar damages arising out of any malfunction of the trial unit during the product Trial period.
