The Importance of Fetal Monitoring

Using fetal monitors, healthcare providers can observe a baby’s heart rate and other vital functions. These medical devices are used during pregnancy and while a mother is in labor. Typically, the fetal heart rate is around 110 to 160 beats per minute and can fluctuate as conditions in the uterus change.1

A fetal doppler monitor allows a clinician to see any changes and whether abnormal heart rates or patterns occur. Such abnormalities can indicate a lack of oxygen or other problems with the baby. These can help doctors determine whether additional testing is needed or if an emergency cesarean section (C-section) is required. Monitoring can be done continuously during labor or if a mother is at high risk, or intermittently at regular intervals for low risk individuals.

Types of Monitoring


Most of the time, fetal monitoring is done externally using a type of stethoscope called a fetoscope. Fetal dopplers are also commonly used. They generate images on a computer screen using sound waves. Less frequently used is internal monitoring, in which an electrode (connected to a catheter) is attached to the baby’s head while it is still in the uterus.

The general types of fetal monitoring include:

  • Auscultation: A fetoscope or doppler transducer is pressed against the abdomen.
  • Cardiotocography: Enables a baby’s heart rate and the mother’s contractions to be monitored using an ultrasound machine. Fetal heart rate and contractions are shown together (as EFM tracings) on a readout to track whether the heart rate accelerates or decelerates.
  • Mobile Electric Fetal Monitoring: Wireless electronic monitors are sometimes used in hospitals. These waterproof devices can be used in the shower. They’re also used when a women is giving birth upright away from the hospital bed.
Fetal doppler monitors are the most popular because fetal heart motion can be heard by everyone in the room. Some dopplers only play the audio, but others have a digital display that shows the baby’s heartbeat and heart rate values. This is generally the most comfortable method and can be used in different laboring positions.

What to Expect During the Monitoring Process


Some types of monitoring require only a stethoscope to be applied to the abdomen. You can get up and walk around in between checks. Doppler monitoring is performed by applying a gel to the skin and moving an ultrasound probe over it to record the fetal heartbeat. A belt with an external tocodynamometer may be put on to record contractions.

With continuous monitoring, sensors are attached to the body with elastic belts and you must either stay in bed or be very close to the monitor. Internal monitoring requires a wire to be inserted through the open cervix. It can only be done if the cervix is dilated and the amniotic sac has broken.
